Vermont Medicaid

How Choices for Care (CFC) works in Vermont

Choices for Care is Vermont's Medicaid long-term-care program. It pays for personal care at home for eligible seniors and adults with disabilities — the same care that would otherwise happen in a nursing facility, delivered where people actually want to live. A&M Home Care Corp is a certified CFC Home Care Provider (Vermont Medicaid Provider #6725852), which means we can bill CFC directly for authorized personal care attendant hours.

Step 1 — Independent Living Assessment

The state conducts an Independent Living Assessment (ILA) that documents the care needs and authorizes hours. A&M coordinates the ILA process.

Step 2 — Caregiver training

Vermont requires client-specific caregiver training tailored to the ILA. If a family member wants to be the paid caregiver, A&M runs the training and certification.

Step 3 — Care starts at home

Personal care attendant hours begin, billed directly to Vermont Medicaid. The client stays home. The paid caregiver — family or otherwise — gets a real paycheck.
